महोदय,

सविनय निवेदन इस प्रकार है कि प्रार्थी के ग्राम................... थाना........... ब्लॉक.................. तहसील............... जिला................... में आने जाने के मेंन रास्तों पर ग्राम के ही कुछ लोगों ने गंदे वस्त्र व घुरा,कुरा कर्कट, गाय का गोवर डालकर का रास्ता बहुत कम कर दिया है! जिससे गांव के हर एक व्यक्ति को अपने वाहन आने व ले जाने में काफी दिक्कतों का सामना करना पड़ रहा है और अधिक गंदगी होने के कारण ग्राम के आसपास के लोगो में भयंकर बीमारी फैल रही है जिसमें गांव के छोटे,बडे बच्चे,व बुजुर्गों में गंभीर किस्म की बीमारी पनपने लगी है!जिससे उनका स्वास्थ्य काफी बिगड़ रहा है! ग्राम के रास्तों की साफ सफाई करवाने के संबंध में पूर्व में ही संबंधित अधिकारियों को उक्त रास्तों से गंदगी साफ करवाने के समय संबंध में कई पत्र दिए जा चुके हैं! लेकिन उन प्रार्थना पत्र के आधार पर अब तक कोई भी साफ सफाई की व्यवस्था नहीं की गई है! जिस कारण प्रार्थी श्रीमान जी को पूर्व प्रार्थना पत्र की छाया प्रति सलंगन कर इस प्रार्थना पत्र को पत्राचार के माध्यम से व स्वयं उपस्थित होकर दे रहा है! ताकि गांव में साफ सफाई की जा सके और गांव बीमारी से मुक्त हो सके !

अतः श्रीमान जी से निवेदन है कि पूर्व के प्रार्थना पत्रों का आज दिनांक................ के प्रार्थना पत्र के आधार पर उक्त ग्राम में साफ सफाई करवाने हेतु आदेश पत्र निर्देशित करने की कृपा करें!

My humble request is as follows that the applicant's village............. thana.............Block........ ............ Tehsil ............. District ............................................ On the main roads, some people of the village have greatly reduced the way by putting dirty clothes and ghur, kura karkat, cow dung. Due to which every person of the village is facing a lot of difficulties in coming and taking his vehicle and due to excessive dirt, a terrible disease is spreading among the people around the village, in which small, big children, and elderly people of the village. A serious type of disease has started developing! Due to which his health is deteriorating greatly. In relation to the cleanliness of the village roads, many letters have already been given to the concerned officials regarding the time of getting the dirt cleared from the above roads. But on the basis of those applications, no cleaning arrangements have been made so far! Due to which the applicant is enclosing the photocopy of the earlier application to Mr. Ji and giving this application through correspondence and by being present himself. So that cleanliness can be done in the village and the village can be free from disease.
